Our registered nurse and social worker care managers help seniors in Minnesota Senior Health Options (MSHO) understand and manage their health care and arrange related support services. MSHO is for people ages 65 and older who are eligible for Medical Assistance and are in Medicare Parts A and B or who have MA only.

Our nurse practitioners and physicians bring primary medical care to you, so you don't have to travel to a clinic for appointments. We make housecalls on select weekdays and can be called 24 hours a day, seven days a week.
